The Value in the Bento Network
The Bento Network has platinum, gold and silver levels. Our Dental 360 plan give members access to two large networks of providers: DenteMax PPO and Connection Dental PPO. This plan works at ANY licensed dentist in the US; however you get the best dental care rates when going to in-network Bento Dentists and Specialists. Each Dental 360 plan will pay the dentist through Bento for any payments. With over 350,000 dental access points nationwide, Bento provides members with easy access to care. Members can find a provider by searching Bento.com or downloading their app from the App Store or Google Play.
Covered Preventative + Diagnostic Services
There is no deductible for preventative and diagnostic services. Each plan level covers a comprehensive evaluation every 60 months, with routine periodic oral exams twice in 12 months. Single tooth x-rays are covered at 100% as needed, with bitewing x-rays once every 12 months, and full mouth x-rays once every 60 months. Routine dental care cleanings are covered at 2 times per year with two periodontal cleanings covered per calendar year.
Restorative Coverage Assistance
Dental 360 basic and major restorative services have a $25 deductible per member per year. Each plan provides 80% coverage for basic restorative services related to fillings, root canal treatment (endodontics), gum treatment (periodontics), prosthetic maintenance, oral surgery and other necessary services. Dental care procedures considered major restorative services include crowns, implants, tooth replacement (prosthodontics) and are covered at 50% of the fee for service.
As a Bento Member, you have access to the entire national Bento Network which includes contracted rates off provider fees with a no balance billing policy when visiting Bento Dentists and Bento Partner Dentists.
