Accidental Death and Dismemberment: This type of policy pays the beneficiary a pre-determined amount if the cause of death is an accident. An AD&D policy is a good add-on benefit to a life insurance policy. Coverage with this policy includes but is not limited to loss of (with detailed requirements):

Additionally, Prescription Assistance, Diabetic Supply Program, and low-cost co-pays may apply.

Term Life: Renewable in 10-year increments until the attained age of 70, our low-cost term life product provides your family with peace of mind should you suffer death. Available in different amounts depending on age, this healthcare coverage benefit is Simplified Issue Group Level Term Life Insurance.

Preventative & Wellness: This healthcare coverage plan provides 100% coverage for ACA mandated preventative services. With guaranteed issue and no exclusions for pre-existing conditions, this plan is geared towards individuals who are interested in having a level of coverage, compared to none. Included in this plan:

  • Four physician visits per year with $25 co-pay
