1. Member Eligibility.

Dental 360 is available to adults age 18 and above who reside in one of the 50 states. Dependent children are covered to the age of 26.

2. Product Overview.

There are three dental care plans to choose from. Members select an annual maximum amount, and which eligible household members will be covered.

• Plan Options: $1,500, $3,000, $5,000 Plan Year Maximums
• Deductibles: $50 Per Covered Person, $150 Per Family
• 100% Coverage for Class 1- Preventative
• 80% Coverage for Class 2- Basic
• 50% Coverage for Class 3- Major
• 6 Month Waiting Period on Class 3- Major Services

3. Access to a Large Network of Dentists.

Guardian PPO Network provides members access to a large community of participating dentists. Members can search online or use the app on the go to find a dentist in their area.

4. Class I, Class II & Class III Coverage.

Preventative and diagnostic dental procedures are included as part of the oral health plan with Dental 360. Additional oral procedures are covered per the policy schedule of benefits.

• Coverage for Class 1- Routine oral exams, x-rays, fluoride treatment, spacers and periodontal maintenance.
• Coverage for Class 2- Amalgam or resin fillings, general anesthesia, extractions and oral surgery
• Coverage for Class 3- Inlays, crowns, root canals, periodontal surgery, repair or recement of crowns and more.

5. Save Money on Vision and Teeth Aligners.

Included with each membership is a vision program offering savings on eyewear at over 20,000 eye care professionals nationwide. Members save on eyeglasses, contact lenses, other retail eyewear items and LASIK. Dental 360 members also save on an at-home impression kit and teeth aligner system with Byte®.

6. Mobile App Available for Convenience.

Find a provider on the go with the Guardian Network app. Members can also use the app to access ID cards from their phone or tablet.
